Safety First.

Our students trust us. That’s why our Release Form/Waiver/Risk Agreement is required to be completed before everyone begins lessons with us. It’s a promise we keep to one another.

The Risk Agreement informs each student and parent about what to expect regarding safety at our martial art school. It protects TheDOJO and it’s employees and also protects you and the other students in our classes.

TheDOJO Release Form is completed using Smart Waiver which is safe and secure.
